Didn't one of the TV show characters lose a hand, and not have it grow back? They can lose them up until first death, then they regenerate... unless they detach in which case its a loss.

If you are writing a fic with a tooth ache, you can postulate slow healing from tooth decay. I don't know of anything in canon to contradict that.
