Can I offer another solution? Sean canceled all of his appointments for Duncan before he met him and got beheaded. It could be when Sean was canceling his appointments, a) one of those appointments was Stephen, and b) he specifically mentioned he was canceling the appointment to see Duncan. Basically puts it from Sean's ear to Stephen's, and gives the opportunity for Stephen to have, "My last words to him were..." guilt. :)
